Only requests for medical records are being processed.  No other forms can be accomodated since WholeKids Pediatrics is closed permanently.  Please send a release of medical records for each child via email to wholekidspediatrics@gmail.com

• Ohio Vaccine Exemption Form;  The Ohio Vaccine Exemption form should be used by families choosing not to vaccinate who need to provide documentation to a school or other entity.  This form is signed and submitted by parents only and does not require the signature of a health care provider.
