Records Request

WholeKids is now permanently closed in Columbus, Ohio.

In order for WholeKids to release your child's records, we must have a release form signed for EACH child.  Please submit your child's release of records attached to an email and send to:  wholekidspediatrics@gmail.com

A $15 fee is applicable for each child's medical record.  When the records release form and the applicable fee has been received, the records will be processed within 10-14 business days.  As stated on the former patient home page, immunization records are not available separately and are part of the whole medical record.
